Before the first day of school on Monday, Aug. 31, it is important that families sign in to Skyward Family Access and complete the required annual forms for each student.

These forms are required annually by the state and also provide families the opportunity to review and update important student information, including phone numbers, emergency contacts, and other records we have on file.

We appreciate your prompt attention to completing this information.

The annual forms include:

  • Annual Health Registration Form
  • Attendance Acknowledgement Form
  • Housing Questionnaire
  • Google G-Suite Education Consent Form
  • Student Device and Software Consent Form

To complete the forms:

  1. Log in to Skyward Family Access.
  2. On the left side of the page, click “2026-27 Required Annual Student Verification Information Update.”
  3. At the bottom of the page, you will see a list of your student(s).
  4. Review each student’s information individually and complete all required forms.
