We know school just ended, summer is in full swing and the new school year might feel far away! But if you would like to get a head start, now is a great time to complete our annual required forms. (If you have recently registered your child for the 2025–26 school year or your student just graduated, you can ignore this message.)
These forms help us prepare for a smooth start in the fall and include:
  • Annual Health Registration Form
  • Attendance Acknowledgement Form
  • Home Language Survey
  • Housing Questionnaire
  • Student Device and Software Consent Form
Before the first day of school on Thursday, Aug. 28, we ask that you log in to Skyward Family Access and complete the annual required forms for your student.
Why
These forms are a state requirement and also give you a great chance to double-check and update important information like phone numbers, emergency contacts and more.
Instructions
Once you log into Skyward Family Access, look on the left of the page and click on 2025-26 Required Annual Student Information Update. At the bottom of the page there will be the list of your student(s). Review this information individually and fill out the annual forms.
